This long thread in Twitter is a collection of anonymous reports from numerous industries about various forms of fraud, abuse, and corruption that within the industry are accepted practice but which remain largely unknown by consumers of products and services. I'm finding I already knew or suspected most of this, as I read through it. Maybe I haven't gotten to the really scandalous stuff yet....

Oh, here's a good one: "The morning I had my right leg amputated I was booked to have my left arm amputated. It required a total of 3 corrections to get the info right. Yup... It went left arm, right arm, left leg, right leg. Each "correction" required the printing of ~100 adhesive labels and 20 pages."
(Also these are not all anonymous after all)

Wait, what? They severed all 4 limbs? I hope the corrections were done before the amputation...

No they finally got it right, apparently, but to do that they had to be *stopped* from doing the other amputations which then generated a ton of paper....
